What Is Testosterone Replacement Therapy?

Testosterone replacement therapy restores testosterone to a healthy range in men who have both the symptoms of testosterone deficiency and confirmed low levels on appropriate blood testing. At HE Clinic we deliver it as the HE T-Shot — a program designed around diagnosis, safety, and follow-up rather than a one-off injection.

That distinction matters. International guidance from the Endocrine Society is clear that testosterone therapy should be reserved for well-documented deficiency, confirmed by symptoms alongside consistently low, accurately measured levels. If your symptoms turn out to have another cause, we'll tell you — and treat that instead.

Recognising the Signs

Symptoms Associated With Low Testosterone

Low energy or persistent fatigue
Reduced motivation or drive
Irritability or low mood
Low or reduced sex drive
Erectile difficulties
Reduced strength or muscle mass
Increased body fat, harder to manage weight
Poor sleep or slow recovery from exercise

These symptoms are common — and testosterone is only one possible cause. Stress, sleep debt, weight changes, alcohol, medications, and other medical conditions can all produce the same picture. That's exactly why testosterone replacement therapy starts with testing, not assumptions.

Test First, Treat Second

How We Diagnose Testosterone Deficiency

A single number on a lab report isn't a diagnosis. Here's the process every HE Clinic patient goes through before any testosterone replacement therapy is prescribed.

1

Consultation

Your symptoms, medical history, medications, sleep, stress, and lifestyle — mapped properly, because several of these can mimic low testosterone.

2

Morning Blood Test

Testosterone peaks in the morning and fluctuates through the day, so timing matters. Testing outside that window is one of the most common reasons for a misleading result.

3

Confirm & Clarify

Borderline or unexpected results are repeated. Where clinically useful, additional markers such as SHBG, LH and FSH help identify the cause, not just the level.

4

Diagnosis & Plan

Treatment proceeds only when symptoms and blood results consistently agree. Your doctor explains the realistic benefits, the risks, and the alternatives.

5

Ongoing Monitoring

Follow-up bloods and review appointments check that you're responding, that levels sit in a safe range, and that no side effects are developing.

Your Blood Panel

What We Test — and Why Each Marker Matters

Your HE T-Shot program includes a blood panel — not just a testosterone number. Each marker has a specific job.

TOTAL TESTOSTERONE

The diagnostic marker

The overall testosterone level in your blood — the primary measurement used, alongside your symptoms, to establish whether deficiency is present and later to confirm you're in a healthy range on treatment.

PSA

Prostate safety screening

Prostate-specific antigen is checked before starting and monitored during therapy. Guidelines call for urological review if PSA rises significantly in the first 12 months of treatment — which is why baseline matters.

CBC

Red blood cell check

A complete blood count tracks haematocrit. Testosterone can increase red blood cell production, so this is one of the key safety numbers watched throughout therapy.

ALT & AST

Liver function baseline

Liver enzymes give a baseline picture of liver health before therapy begins and a reference point for follow-up testing.

Realistic Expectations

When Do the Benefits Appear?

Different benefits arrive on different schedules — libido responds within weeks, body composition takes months. Knowing this up front is the difference between staying the course and quitting early.

Start 1 mo 3 mo 6 mo 9 mo 12 mo+ Sex drive & libido first to respond — around 3–6 weeks Energy & mood builds over the first months Erectile function may take up to 6 months Muscle & strength with training & nutrition Body fat reduction gradual, continues beyond a year Bone density slowest — measured in years Typical window of change Continued benefit with ongoing therapy

Typical time courses reported in clinical research for men with confirmed testosterone deficiency. Individual response varies — your doctor will review your progress at each follow-up.

Treatment Options

Long-Acting or Short-Acting?

Testosterone is given by injection, and the main difference between options is how often you need one. Your doctor will recommend the right approach based on your diagnosis, lifestyle, and medical suitability.

OUR STANDARD PROGRAM

Long-Acting Injection

Approximately every 3 months
  • Just a few injections per year once your dose is established
  • Steady, sustained levels between doses — no weekly routine
  • Convenient if you travel or have an unpredictable schedule
  • The basis of our 90-day HE T-Shot program
ALTERNATIVE

Short-Acting Injection

Typically every 2–4 weeks
  • More frequent dosing schedule
  • Levels can be adjusted — or treatment stopped — more quickly
  • Lower cost per injection
  • Useful when starting out or when fine-tuning your dose

Specific products are prescribed and discussed during your consultation, subject to clinical indication and availability.

Honest Medicine

When TRT Is Not the Right Answer

Testosterone replacement therapy isn't suitable for every man, and a clinic that says otherwise isn't being straight with you. Your doctor will screen carefully — and recommend against treatment where it isn't appropriate.

Planning to have children Testosterone therapy suppresses the body's own sperm production and can affect fertility. If you're planning a family in the near term, we'll discuss alternatives first.
Prostate concerns An elevated PSA, a prostate abnormality, or a history of prostate or breast cancer requires urological assessment before testosterone is considered.
Certain heart & blood conditions Uncontrolled heart failure, a recent heart attack or stroke, elevated haematocrit, or clotting disorders all need to be addressed before therapy.
Untreated sleep apnoea Severe untreated obstructive sleep apnoea should be managed first — and treating it often improves the very symptoms that prompted the visit.

Common Questions

Testosterone Replacement Therapy — FAQ

My blood test came back "normal" — do I still need treatment?

Not necessarily — but a single normal result doesn't automatically rule deficiency out either. Testosterone is most accurately measured in the morning, levels fluctuate day to day, and recent illness, poor sleep, or stress can all skew a reading. Borderline or unexpected results are repeated, and additional markers such as SHBG, LH and FSH may be used to clarify the picture. Treatment starts only when symptoms and bloods consistently agree.

What is a healthy testosterone level?

Like cholesterol or blood pressure, testosterone has reference ranges — but those ranges vary between laboratories and testing methods, which is why a number alone means little. Your doctor interprets your result against your symptoms, your history, and repeat morning testing. On treatment, the aim is a safe mid-range level, confirmed by follow-up bloods rather than guesswork.

How soon will I feel a difference?

It depends on the benefit. Libido and sex drive typically respond first, often within 3–6 weeks. Energy and mood build over the first few months. Erectile function can take up to 6 months, and changes in muscle mass and body fat develop gradually from around 3 months onward — accelerated considerably by training and nutrition. Bone density is the slowest of all, measured in years.

Will TRT affect my fertility?

Yes — this is important. Testosterone therapy suppresses your body's own testosterone and sperm production, which can reduce fertility. If you're planning to have children in the near term, tell your doctor at consultation: we'll discuss alternative approaches rather than starting testosterone.

Is testosterone therapy safe?

When prescribed for properly confirmed deficiency and monitored appropriately, testosterone therapy has a well-established safety profile. The safeguards matter: screening before treatment, PSA and haematocrit monitoring during it, and follow-up reviews to confirm you're responding and levels sit in a safe range. It isn't suitable for everyone — see the section above on when TRT isn't the right answer.

Is TRT an anti-aging treatment?

No, and we won't sell it as one. Testosterone replacement therapy treats a diagnosed medical condition — testosterone deficiency. It isn't a guaranteed longevity or anti-aging therapy, and it isn't a substitute for sleep, training, and nutrition. Where your symptoms have another cause, treating that cause will serve you better.

Do I have to stay on it for life?

Not automatically. Duration depends on why your levels are low. Some men have a reversible or contributing cause that, once addressed, improves their own production; others have a permanent deficiency where ongoing therapy makes sense. Your doctor will discuss what's realistic in your case and review it at each follow-up.
