The Technique

What Is DHI?

DHI stands for Direct Hair Implantation. Follicular units are harvested from the donor area at the back and sides of the scalp, then placed into the thinning area using an implanter pen, commonly called a Choi implanter.

The extraction step is the same as standard FUE. The difference is the implantation: with an implanter pen, creating the recipient site and placing the graft happen in one continuous motion, which reduces handling and limits the time grafts spend outside the body.

Neither technique removes a strip of scalp, so neither leaves the long linear scar associated with older FUT strip surgery.

Most patients are surprised by the shedding phase, so here is the honest sequence. Transplanted hairs falling out in the first weeks is normal and expected, not a sign the procedure failed.

About shock shedding

Transplanted hairs commonly fall out in the first weeks after the procedure. The follicle stays in place, it simply enters a resting phase before producing new hair. This is the stage where patients worry the transplant has failed, so it is worth knowing about before you book rather than discovering it afterwards.

Your hairline and density plan are agreed, then the recipient area is shaved. The larger the area, the more shaving is needed.

Aftercare

Recovery and What to Avoid

My transplanted hair is falling out. Has it failed?

Almost certainly not. Shock shedding in the first weeks is normal and expected. The follicle remains in place and enters a resting phase before producing new hair. Growth typically begins around 3 to 4 months, with results visible at 6 months and the full picture at around 12 months.

Will a transplant stop my hair loss?

No, and this matters. A transplant relocates existing follicles, it does not treat the underlying cause. Without a maintenance plan, non-transplanted hair can continue to thin around the new growth. Your doctor will discuss whether ongoing treatment makes sense for you.

Do I have enough donor hair?

Donor supply at the back and sides of the scalp is the main factor determining how much coverage is achievable. It is the first thing assessed. Send clear photos from the front, top, sides and back for an initial view, or come in for a full assessment.

How long does the procedure take?

Do I have to shave my head?
