BoCox Results: How Long Does Penile Botox Take to Work?

In This Article

BoCox results are one of the first things men want to understand before choosing penile botulinum toxin treatment for erectile dysfunction. Unlike sildenafil or tadalafil, BoCox is not designed to create an erection on demand within hours. Human research suggests that changes may begin to appear over the first few weeks, with several studies assessing erectile function at two, six and twelve weeks after treatment. At HE Clinic Bangkok, BoCox results should therefore be judged over time and against a man’s starting level of erectile dysfunction, medication response and overall vascular health.

What Is BoCox?

BoCox is HE Clinic’s doctor-led use of botulinum toxin for selected men concerned about erectile function, firmness or sexual performance. Depending on the treatment plan, it may be used alone or combined with PRP. HE Clinic BoCox treatment

In erectile-dysfunction research, intracavernosal botulinum toxin is being studied because relaxation of cavernosal smooth muscle may help reduce resistance to penile blood inflow and support erection quality in some men.

This remains an emerging use rather than an established first-line ED treatment. That distinction matters when discussing BoCox results because published studies are still relatively small and treatment protocols differ.

When Can BoCox Results Start to Appear?

The best human evidence suggests that measurable changes can appear within the first few weeks.

In a double-blind randomized controlled trial involving 70 men whose ED had not responded adequately to PDE5 inhibitors, researchers assessed erection hardness and penile Doppler measurements two weeks after a single botulinum toxin injection. The treatment group showed significant improvements in erection hardness and penile blood-flow measurements compared with placebo. Read the randomized controlled BoCox study on PubMed

The same study continued to assess sexual-function outcomes at six and twelve weeks. At six weeks, 53% of men in the treatment group were able to achieve an erection hard enough for penetration, compared with one man in the placebo group.

That does not mean every patient should expect the same response. But it gives a useful framework: BoCox results may begin to become measurable at around two weeks, while broader sexual-function changes may be clearer over the following month or two.

Why Assess Results Around Six Weeks?

HE Clinic’s existing BoCox material says fuller effects are commonly assessed at around six weeks.

That fits reasonably well with the randomized-trial evidence. Two-week measurements can show whether erection hardness or penile blood flow has changed, but six weeks gives more time to judge whether those physiological changes are translating into erections that are useful during sex.

For that reason, BoCox results should not be judged the day after treatment or after one sexual encounter. An early good erection also does not prove that the treatment will provide a sustained effect for months.

How Long Can BoCox Results Last?

Current evidence suggests that the effect of penile botulinum toxin is temporary.

A 2026 systematic review of botulinum toxin for male sexual dysfunction reported that improvements in erectile function in published studies could persist for up to six months. Read the 2026 systematic review on PubMed

HE Clinic’s current BoCox service information similarly describes the effect as lasting for several months before gradually wearing off.

However, it would be misleading to promise that all BoCox results will last six months. Duration may differ substantially between men, and research does not yet provide a reliable formula for predicting exactly how long an individual response will last.

What Do Clinical Studies Actually Show?

The evidence is promising but still developing.

A 2022 systematic review and meta-analysis found that intracavernosal botulinum toxin improved erection hardness and peak systolic velocity, a penile Doppler measure of arterial blood flow, compared with placebo. The researchers cautioned that the available studies were small and heterogeneous. Read the 2022 systematic review and meta-analysis

A 2025 systematic review and meta-analysis found that at least 40% of patients achieved a clinically meaningful improvement in erectile-function scores at one month or later. When three botulinum toxin formulations were compared across included studies, the overall response rate was reported as 77.5%, although definitions of response varied. A broader 2026 systematic review reported erectile-function improvements in approximately 41% to 57.4% of patients across the studies it analysed, with benefits reported for up to six months.

These different figures explain why BoCox results should never be reduced to one advertised success percentage. Studies recruited different men, used different doses and defined “response” differently.

Do Results Depend on How Severe ED Is?

Probably.

Some studies suggest better outcomes in men with mild-to-moderate ED, while other trials have specifically studied men with refractory ED who had failed oral PDE5 inhibitors and still found meaningful responses in some participants.

A man with predominantly vascular ED may respond differently from someone whose erection problems are caused mainly by severe diabetic neuropathy, major pelvic nerve injury or advanced arterial disease.

What If Viagra or Cialis Has Stopped Working?

Some of the strongest research has involved men who responded poorly to PDE5 inhibitors such as sildenafil or tadalafil.

In the randomized trial above, men with PDE5-inhibitor-refractory ED showed improvements in erection hardness, penile blood flow and sexual-function outcomes after botulinum toxin compared with placebo.

This makes BoCox an interesting potential option for selected non-responders. It does not mean men should stop ED tablets without medical advice, and BoCox results should always be interpreted in the context of the other treatments a man is using.

Does PRP Change the Expected Results?

Potentially.

HE Clinic currently offers BoCox both without PRP and as a combined BoCox + PRP treatment. View HE Clinic’s current BoCox treatment options and pricing

The two therapies are biologically different.

Botulinum toxin is intended to influence local smooth-muscle tone, while PRP contains platelets and growth factors and is used as a regenerative treatment. If a patient receives both therapies together, any improvement cannot automatically be attributed to botulinum toxin alone.

Published botulinum toxin trials generally investigate the toxin itself under controlled conditions. Real-world BoCox results after a combined treatment may therefore not map perfectly onto those studies.

How Should BoCox Results Be Measured?

“Did it work?” is not always a useful enough question.

A better assessment may include:

  • erection hardness;
  • ability to maintain firmness;
  • successful penetration;
  • frequency of successful sexual attempts;
  • morning erection quality;
  • response to sildenafil or tadalafil;
  • validated questionnaires such as SHIM or IIEF;
  • and, where appropriate, penile Doppler measurements.

The randomized trials use several of these measurements because erection treatment is not adequately assessed from one successful or unsuccessful sexual encounter.

Why Do Results Vary Between Men?

Erectile dysfunction has many possible causes, including vascular disease, diabetes, medication, smoking, nerve damage, pelvic surgery and psychological factors.

Botulinum toxin does not correct every one of those problems.

BoCox results may therefore depend on the underlying cause and severity of ED, baseline penile blood flow, cardiovascular health, diabetes control, medication, previous treatment response, dose and whether another therapy such as PRP is used at the same time.

Are BoCox Results Permanent?

No.

Current evidence does not support describing penile botulinum toxin as a permanent cure for erectile dysfunction. The effect gradually diminishes.

If a man experiences useful improvement and later notices it wearing off, the doctor can discuss whether repeat treatment makes sense.

Patients should be cautious of any provider promising permanent BoCox results from a single treatment.

What If There Is No Improvement?

A disappointing response does not automatically mean a higher dose or immediate repeat treatment is appropriate.

The next step should be to reconsider the cause of the ED. That may involve medication review, cardiovascular risk assessment, diabetes evaluation or discussion of other options such as focused shockwave therapy, PRP or other specialist ED treatments.

The goal should be to treat the man’s erectile dysfunction appropriately, not simply keep repeating one procedure.

Are There Side Effects While Waiting for Results?

Published clinical research has generally reported a favourable short-term safety profile.

Recent systematic reviews describe mild, temporary penile pain as the most commonly reported local adverse effect, while serious systemic complications were not reported across the included studies. One case of priapism was reported in the 2025 systematic review.

HE Clinic also lists temporary soreness, swelling, bruising and small amounts of bleeding at injection sites as possible effects after treatment.

Men should follow HE Clinic’s dedicated BoCox aftercare guidance and contact the doctor if symptoms are severe, worsening or unexpected.

When Should You Decide Whether BoCox Has Worked?

Current evidence suggests it is more sensible to judge treatment over weeks rather than days.

Two weeks may provide an early indication. Six weeks is a useful point for assessing erection hardness, reliability and sexual performance. Studies have also followed men at twelve weeks and later to evaluate whether benefit persists.

This evidence-based timeline is a more useful way to discuss BoCox results than promising an instant change or an exact date when every patient will notice an effect.

BoCox Results: The Bottom Line

BoCox results can begin to become measurable within the first few weeks after penile botulinum toxin treatment, while fuller changes in erection quality may be easier to judge around six weeks. Human randomized trials and systematic reviews report improvements in erection hardness, erectile-function scores and penile blood-flow measurements in selected men, including some whose ED did not respond adequately to oral medication.

However, BoCox results vary considerably. Current research remains limited by relatively small studies, different doses, different patient populations and different definitions of treatment success. The effect is also temporary, with published studies reporting benefits that may persist for several months rather than permanently.

At HE Clinic Bangkok, the most useful way to evaluate BoCox results is to start with the cause and severity of your ED, establish realistic goals and then assess changes in erection hardness, reliability and sexual function over an appropriate follow-up period.
